IMPORTANT
If you have already secured an appointment for a procedure/s with us, please print the relevant Patient Forms, and go through the instructions carefully. The Consent Forms require your signature and it is MANDATORY for you to bring the SIGNED CONSENT FORMS along with you ON THE DAY OF THE PROCEDURE.
